During the first visit to Carolina Foot & Ankle, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor.  We accept most insurances.

We have made exciting updates to our Electronic Medical Records System. If you are a patient going to one of our offices, please use the Patient Portal – Online Forms button below to direct you to a portal to fill out your information online quickly. Doing this ahead of your visit will make your first visit seamless.

Spanish and Large Print New Patient Form Downloads

If you are a new patient in need of Large Print Forms or Spanish Forms, please complete all pages of the patient forms below before attending your first appointment. The forms may be filled out on your computer but will need to be printed and brought with you to your appointment.

Arriving prepared for your appointment will help us to ensure that your needs, as a new patient, are understood and that the process can be expedited appropriately. You only need to fill out one set of forms.
